Birth and Death Certificates

Birth or death certificates may be obtained either through San Juan Basin Health Department or directly through the Colorado Department of Public Health & Environment. Individuals can come into San Juan Basin Health's Durango office for birth and death certificates: 8:00 a.m. to 11:15 a.m. and 1:30 t o 3:00 p.m. For more information on obtaining either birth or death certificate at San Juan Basin Health, please click below:
