Traveling west up the hill on Sawyer Drive you will see a two story building on your right, just past the Qwest building. There will be a sign by the road identifying the building as the Columbine Center.
Please park in the lower parking lot, enter the front doors and either take the elevator to your right up to the second level, or walk up the staircase on your left to the second level.
You will see the Front Desk to SJBHD on your left as you come off the elevator or staircase.
Sample bottles to collect water are available at the Front Desk and must be returned within 30 hours of collection. Testing is done immediately and results are usually mailed within a day or two.
The cost is $25.00 when you return the water for testing. SJBHD does not invoice for this service.

Yes. Travel immunizations are arranged on an appointment only basis. Please call 970-335-2001 to speak to our Travel Immunization nurse. It is highly recommended that you call 6-8 weeks prior to travel. To learn more about travel immunizations and costs, click here.
Routine childhood and adult immunizations are offered by appointment. Please call 970-335-2013 to schedule an appointment in La Plata County or 970-264-2409 in Archuleta County.
San Juan Basin Health offers reproductive health services for males, females and teens. These include:
- Annual Pap Smears
- Birth Control
- STD/STI Testing and Treatment
- Sexual Health counseling
- Women's Wellness Connection (free clinical breast exams, mammograms, pelvic exams and Pap tests to qualifying women)
Next day appointments for these services can be made by calling 970-335-2013 in La Plata County or 264-2409 in Archuleta County.
We do not provide direct services. However, San Juan Basin Health Department houses the Options for Long Term Care/Single Entry Point program which assists individuals to stay as independent as possible in their own home. They do this through coordinating care, case management, and accessing resources including Home and Community Based Services (HCBS).

No. San Juan Basin Health does not offer free health care services. In some circumstances, sliding fee scales can be applied. We have specialists on staff who can help in applying for various programs such as Medicaid or CHP+ , CARES card, and EPSDT. To learn more about these programs call 970-335-2022.
