San Juan Basin Health Department provides immunizations for international travelers by appointment. Please schedule appointment 6 to 8 weeks prior to travel by calling 335-2013. Please review the Travel Immunization Factsheet for available vaccines and prices. You can visit Tripprep.com to look at vaccine recommendations and other important travel information specific to your destination.
Appointments consist of reviewing traveler's immunization history, destination health risk report and vaccination recommendations, as well administration of vaccinations. Malaria risk is discussed with appropriate guidance for securing a prescription for anti-malarial medications from your local physician. A yellow “International Certificate of Vaccination” is issued at time of vaccinations.
Our Travel Health consultation service is an Office Visit with an RN with specialized training in Travel Health and the associated fees are $85 for a visit for one person, $100 for two, and $50 for each additional person at time of appointment. Payment is due at time of service: we accept credit cards, cash or checks. SJBHD does provide some insurance billing; please inquire when scheduling your appointment. You are solely responsible to determine whether your health insurance will “reimburse” you for the costs of vaccine services. We do not provide pre-authorization services.
Please bring your previous vaccination records with you to the appointment. The nurse will base recommendations for you on many factors, including vaccines you have previously received. If you have a Yellow Certificate of International Travel Vaccine record, please bring it to your appointment. If you have never been vaccinated for international travel and don’t have a Yellow Certificate, bring your most recently acquired vaccination records to the appointment i.e., your last Tetanus booster, or a school/college vaccine record. Before your scheduled appointment, PLEASE locate your immunization history and be aware that your appointment may need to be rescheduled without this information.
