We provide routine immunizations for children and adults by appointment.

Durango: Call 335-2013 (Mon-Thurs 8:00 to 5) to schedule an appointment. Pagosa Springs: Call 264-2409 for an appointment (Immunizations available Thursdays).

San Juan Basin Health is a provider for Vaccine For Children (VFC) program – Federally funded to provide vaccinations to children regardless of ability to pay. Your children ages 0 through age 18 may qualify based on these criteria: 1) Medicaid; 2) Native American/Native Alaskan 3) No Health Insurance coverage; 4) Under-insured (health insurance does not cover vaccine services or there is vaccine cap) A $21.65 fee per vaccine helps supports the costs of providing the free vaccine products and is considered a donation. No child is ever turned away for inability to pay. Adults who qualify (see above criteria) may be eligible for low cost vaccines as well.
